Neuro Otology and Vestibular Laboratory

Neuro-Otology is a sub speciality of ENT. The Neuro otology and Vestibular lab was established in the Dept of ENT, Head and Neck Surgery in November 2020. It is a dedicated centre for managing patients suffering from Vertigo and other diseases causing disequilibrium. Vertigo and dizziness are common presenting symptoms to clinicians of all specialties. It has a lifetime prevalence of around 20- 30%. The diagnosis of Vestibular disorders has seen tremendous advancement with the advent of new technology and equipment There are over 40 common inner ear disorders and those of the Central Nervous System which can lead to vertigo. The comprehensive management of these disorders includes detailed history, appropriate investigations, rational treatment and rehabilitation strategies. The first step for successful treatment is establishing a diagnosis. Vertigo and dizziness are symptoms like headache, nausea or fever and relate to specific pathologic conditions. Hence interpretation of history with clinical and diagnostic tests is important.

The Neuro otology Clinic will function on Tuesday. Thursdays and Saturdays from 10 am to 1pm in the ENT Department, Ist floor of the SRM Hospital. It will be jointly managed by suitably qualified professionals from Dept of ENT, Audiology and Neurology. Patients are scheduled by prior appointment only. Vestibular Laboratory

In order to evaluate the Vestibular system and enable the clinician to arrive at a precise diagnosis tests / investigations are usually done in the newly created Vestibular Lab. These tests are

Dynamic Visual Acuity (DVA)

It is the best test to diagnose Vestibulotoxicity and peripheral vestibulopathy usually caused by ototoxic drugs. It can also be used as a rehabilitation tool and to assess outcome of rehabilitation

Computerized Stabilometry (Posturography)

Stabilometry helps in the functional evaluation of the postural control system in its steady state behaviour. It measures the outcome of vestibular rehabilitation by assessing patient’s balance control on the postural sway velocity.

Rhythmic Weight Shift (RWS)

To quantify the patient’s ability to rhythmically move their COG from left to right and forward to backward between two targets at different speeds.

Subjective Visual Vertical (SVV)

It is used to evaluate the otolith system responsible for the perception of verticality. It helps to diagnose various otolithic disorders, chronic dizziness, compensated vestibular disorders, differentiate peripheral vestibular disorders from the central vestibular disorders and assess the effect of rehabilitation in vertigo patients.

Craniocorpography (CCG)

It is an objective investigation to evaluate the vestibulospinal reflex. The Romberg’s test, Tandem walking and Unterburger / Fukuda test are done.

Videonystagmography (VNG)

It is non-invasive, accurate and reliable way of evaluating vestibular and oculomotor systems. It evaluates peripheral and central vestibular functions through spontaneous nystagmus with and without optic fixation, gaze evoked nystagmus, saccades, optokinetic test, caloric test, head shaking test, smooth tracking with various frequencies, positional tests, Valsalva and hyperventilation induced nystagmus and skew deviation.

Video Head Impulse Test (vHIT) with VOR Suppression Test

The vHIT enables to test all 6 semi-circular canals at high frequency as well as the saccades strategies (overt and covert). The important features are

  • Quick test: In 10 minutes, all 6 semi-circular canals can be tested.
  • Nine axis motion sensor accurately measures the head movement allowing for simultaneous comparison of head and eye movement
  • Monocular High Frame rate camera with 200 fps to detect fast eye movements
  • Lightweight goggle design to reduce slippage.
  • Built-in lasers that provide stimulus for calibration and VOR testing.
  • Fully integrated and computerized with real time recording of all parameters
  • Videos of the eye movement can be viewed after the test

Vestibular Rehabilitation Therapy (VRT)

VRT is used for treating vestibular disorders. The Cawthorne-Cookesy exercises, which are now over 60 years old, do not meet the requirement of vestibular compensation in many patients. It is important to identify the areas of dysfunction to tailor rehabilitation strategies. Rehabilitation needs to be progressive and adapt to the needs of the patient. It cannot be “One size fits all” approach. Virtual Reality based rehabilitation is making rehab more real and able to expose the patient to real life situations in the protected environment of the Vestibular rehab clinic.

Vertigo is a common multisensory syndrome. A thorough diagnostic assessment helps to establish the pathophysiology of the disorder. In recent times, diagnostics of vertigo has undergone a paradigm shift due to new diagnostic possibilities. The introduction of modern vestibular test procedures in clinical practice had led to assessment of all components of the vestibular organ. Combined with established methods, a frequency-specific assessment of the function of vestibular reflexes is possible. These developments have enabled the clinician to manage vertigo better by quicker evaluation with this diagnostic equipment and customise the rehabilitation therapy.

The Vestibular Lab will function on all working days from 10 am to 1pm. It is located in the Department of ENT, Ist floor, Hospital ‘B’ Block. Appointments are made with the Nurse in charge of ENT OPD or by telephone on +914447432386

Comprehensive Cleft Care Program – SMILE TRAIN Project

A dedicated facility for the comprehensive management of children born with Cleft lip and palate has been initiated in the Department of Plastic and Reconstructive Surgery SRM Medical College Hospital and Research Centre, Kattankulathur, The cleft care is carried out by a multidisciplinary team of Anaesthesiology Peadiatrics, ENT, Oral Maxillofacial Surgery, Orthodontics, Speech, Language Pathology and Audiology, Clinical Psychology, Community Medicine, Nutrition. Nursing and Psychology. Other support services are Dept of Radiology, Blood Bank etc.

In pursuit of excellence, the cleft care program of SRM Medical College Hospital and Research Centre, Kattankulathur, is to partner with ‘Smile Train’ an international organisation devoted exclusively to the noble cause of helping provide totally free treatment for any child born with cleft lip and palate, through credentialed hospitals worldwide. This partnership would help to establish the cleft care program in our institution as a “centre of excellence” for provision of safe and quality care delivered on par with international standards. Smile Train – India, has consented to consider our application for partnering following the credentialing process.

The most common deformity of the face is cleft lip and/or cleft palate of one or both sides. This congenital deformity happens during the 8th to 12th week of foetal development. The possibility of early diagnosis and treatment planning starts from prenatal diagnosis through postnatal first consultation with a Paediatrician and Cleft palate Surgeon. In a cleft team, the feeding issues can be easily addressed by a qualified nurse who trains the mother and family members to use specialized feeding bottles and correct feeding postures and techniques to provide necessary nourishment. A healthy child with cleft lip/palate is often ready to undergo a correction procedure for cleft lip as early as 3 months and 9 months for a cleft palate.

In addition to correction of the cleft, issues related to speech, hearing, dentition, facial growth, etc. will have to be dealt with in a child with cleft during their growth and development. Later secondary reconstructive surgeries including Alveolar Bone Grafting (ABG), Rhinoplasty, jaw corrective surgeries, etc. may be required at different ages from adolescence through adulthood.

The need for long-term, multidisciplinary intervention has driven us to establish a comprehensive cleft care program with an opportunity to evolve as a Centre of Excellence. The ultimate objective is “to provide all the required care for a child born with cleft lip palate and craniofacial deformity addressed under a single roof by specialised professionals, enabling smiles across faces”.

Molecular Diagnostic Center

SRM Medical College Hospital and Research Centre has a state of the art Biosafety Level 3, NABL accredited Molecular diagnostic center which was inaugurated on September 2020. COVID (SARS CoV2) Real time PCR- qualitative assay and Hepatitis C RT PCR is performed at the laboratory. A total of 11,358 samples were processed from January 2021 to June 30th 2021 for effective management of COVID 19 second wave. 46 samples for HCV RNA RT PCR were processed in the first six months of the calendar year 2021.

We have been selected as one of the sites for COVAXIN clinical trial which is currently in Phase 3. We have processed 2100 samples as of June 30th 2021 as part of this trial.

The laboratory is currently in collaboration with WHO on two study projects. Study 1 is aimed at evaluating the seroprevalence of COVID Total Ig antibodies among healthcare workers in India.

Study 2 is aimed at analyzing the effectiveness of COVAXIN and COVISHIELD vaccines against severe COVID 19. It is a multicentric hospital based – case control study. We have recruited 87 cases and 243 control subjects as of June 30th 2021 in the study.
